Vital and essential medications (Vital and Essential Drugs; until 2011 - “Vital and Essential Drugs”, Essential and Essential Medicines) - a list of medicines approved by the Government of the Russian Federation for the purpose of state regulation of prices for medicines. The task of state regulation of drug prices is to increase accessibility medicines for the population and medical institutions.
Vital and Essential Drugs List contains a list of medicines under international nonproprietary names and covers almost all types of medical care provided to citizens of the Russian Federation under state guarantees, in particular, ambulance, hospital care, specialized outpatient and inpatient care, and also includes a significant amount of medicines implemented in the commercial sector. In addition, the List of Vital and Essential Drugs serves as the basis for the development of regional lists of constituent entities of the Russian Federation and formulary lists of medicines of stationary medical organizations.
It is formed using the international Anatomical Therapeutic and Chemical Classification of Medicines - ATX (Eng. ATC - Anatomical Therapeutic Chemical classification system).

The list of vital and essential medicines is expanding every year due to the emergence of new highly effective drugs that are in demand in domestic health care for emergency medical care.
Recently in Vital and Essential Drugs List drugs were included to help patients who suffer from cancer. These are such drugs as: Erlotinb, Lomustine and Imatinib.
The range of high-performance hormonal drugs was also expanded by adding Fludrocortisin, Nandrolone and Leiprorelin to the List of Essential Drugs.
Vital and Essential Drugs List for the first time replenished with immunostimulants, which are so necessary in domestic health care - Tiloron, Meglumina, Azoximer.

Lists similar to the List of Vital and Essential Drugs are in many countries of the world. They must be in all medical institutions. They are usually made up without taking into account the formulary lists and treatment standards. It’s just that the best experts are engaged in their development, given the possibility of the course of a disease. Standardization in treatment causes a lot of controversy among doctors. It would seem that the complete lack of standards can lead to diagnostic, and then to medical errors, which are not so easy to fix later. But on the other hand, many doctors do not agree to treatment only by standards from the point of view of their practice. The List of Vital and Essential Drugs gives doctors some freedom of action, as it is compiled according to the international generic names of medicines. This means that drugs are created strictly according to the chemical formula. Only problem lies in the fact that for each formula there can be not one trade name, and not even two, but tens. That's why the doctor can choose the drug at his discretion.
Every year it is necessary to check the List of Vital and Essential Drugs, this is necessary for its expansion by approximately two dozen dosage forms and drugs.
A few words about drugs
Due to the complications caused by the uncontrolled use of drugs, hundreds of thousands of people die all over the world. One of the causes of death can be called adverse reactions that occur after the use of medicines. The doctor, when prescribing the medicine, cannot accurately determine the risk of side effects. It is believed that more than 3 drugs should not be taken at the same time. Any medicine has either an international nonproprietary name (INN) - a name recommended by the World Health Organization, or a grouping name - a name in the absence of an INN. In this case, the manufacturer must also assign a trade name. For example, ibuprofen is an INN, and on sale there are such trade names: Mig (produces Berlin-Chemie), Nurofen (from Reckitt-Benkizer), Next (production Otisifarm). Different manufacturers give drugs with the same active substance different trade names to identify the drug of a particular developer.
The state register of all registered drugs is maintained by the Ministry of Health of the Russian Federation on the official website.
Where to find the list of essential drugs for 2017
In order to ensure priority needs of healthcare, prevention and treatment of diseases, including those prevailing in the structure of morbidity in the Russian Federation, the Ministry of Health maintains a list of so-called special drugs - Vital and Essential Drugs. It is approved annually. Below you can download the updated list of Vital and Essential Drugs for 2017.
Since 2012 the list is called vital drugs and not vital drugs, i.e. drugs, not in the broader concept of drugs. And, although both abbreviations are still in use today, it is important to remember that the second option is not entirely correct.

Features of Vital and Essential Drugs Purchases
Particularly noteworthy are the features of conducting orders of this category of drugs:
1. Additional reasons for the rejection of participants: the maximum price of vital drugs is not registered or the proposed price exceeds their maximum selling price under the conditions specified in clause 2, part 10, article 31.
2. Mandatory indication in the INN documentation (in their absence - chemical grouping names), and when conducting a request for proposals for procurement by decision of the medical commission for one patient during the treatment period, the trade name may be contained in the documentation.
3. The approved Rules for the formation of the list of drugs, the purchase of which is carried out in accordance with their trade names (Decree of November 28, 2013 N 1086), but the list has not yet been created.
4. Restrictions on the purchase of drugs of foreign origin in accordance with the Resolution of 30.11.2015 N 1289.
